Pursuant to the Communiqué on the Procedures and Principles of Application to the Data Controller, it is mandatory for the Data Subject's application to include their name, surname, signature if the application is in writing, ID number (passport number if the applicant is a foreigner), residential or workplace address for notification, electronic mail address for notification if any, telephone number and fax number, and information regarding the subject of the request.

The Data Subject must state the requested matter clearly and comprehensively in the application containing the explanations regarding the right they will use and request to use. Information and documents regarding the application must be attached to the application.

Although the subject of the request must be related to the person of the applicant, if acting on behalf of someone else, the person making the application must be specifically authorized in this regard and this authority must be documented (special power of attorney).
